- Full Name (First, Middle / Middle Initial, Last)
- Location (City, State / Province, Country)
- LinkedIn® Profile URL (if available)
- Short Bio (250 words or less)
- Your Pitch for Why They Should Win the Award (150 words or less)
- A Profile Photo
In addition to sending your nominee’s info, please also supply the following info about yourself (we will not share your information with anyone; click here for more info on our privacy policy):
